Transcript Request Form.pdf Applicants to MJI Study Abroad Program must verify completion of High School.

Study Abroad Release Form.  Applicants must read and sign the release form. If the applicant is under 18 years old, a parent or guardian must sign where indicated.

Release of Information Form Students should also complete and sign the Release of Information Form to authorize exchange of your student information with a parent and/or others designated.

(2nd Year Students) Extended Study Abroad Form.  Students planning on spending a second year studying abroad should complete and sign this form
